Who will take care of my aging loved one?
At Trinity Care, we offer a coordinated program with skilled nursing services from CNA’s, LPN’s, RN’s and trained therapist. Services are designed to provide a vibrant social community to adults who need supervised care in a safe place outside the home during the day. Adult patients receive care and monitoring on a regular basis, in the privacy of our Nurse’s clinic.
What Health Care Services does Trinity Care offer?
Our Licensed Nurses have the knowledge and skills needed to maintain quality care in a medical-social setting. In addition to monitoring and treating all aspects of health management for our participants, our nurses administer medication, weight and vital sign monitoring, diabetic care, feeding tube care, and dressing changes. Regular contact is made with the participants physician as needed to coordinate care with family members and other health care providers as necessary.
Does Trinity Care provide transportation?
Trinity Care strives to make life easier by offering transportation services from a participant’s home location to our center. Transportation services include vans equipped to handle walkers and wheelchairs as well as ambulatory participants.
Who is eligible for adult day care?
Individuals who require supervision during the day due to cognitive, physical, psychological, or social reasons. Comprehensive evaluations are completed on admission to determine the Level of Care required.
How are services paid for?
We accept long-term care insurance, private pay, Veterans administration programs and funding through the Area Agency on Aging. Contact us today so we may assist you in identifying the appropriate funding options.
How often should I visit Trinity Care to become a regular participant?
We prefer participants to attend at least 2 days per week regularly. Services are offered based on the caregivers needs however, most participants attend at least 4 days per week.
What should I do if my family member doesn’t want to participate in adult day care activities?
While we encourage our participants to take part in our recreational programs, they are never obligated to do so. For those participants who prefer less interaction with large groups, we offer one-on-one activities, Brain fitness, or arts and crafts. Individual activity programs are available and designed to stimulate the mind without the pressure of socialization.
How do adult day care fees compare to other aging services fees?
